What is a DOT Physical?

A Department of Transportation (DOT) physical, is a medical examination designed to ensure that you meet the necessary standards to operate a commercial motor vehicle (CMV). CMVs include semi-trucks, buses, and vehicles that transport hazardous materials. Operating these vehicles comes with significant responsibilities, and employers need to confirm that you can perform these duties safely.

For instance, you must be able to hear surrounding noises, read road signs clearly, and remain alert. Various health conditions or medications could impair these abilities or increase the risk of medical emergencies, which could be dangerous while driving.

The Department of Transportation establishes the guidelines for who needs a DOT physical, generally mandating it for those who drive CMVs across state lines. Individual states also have specific regulations for intrastate CMV drivers, so you might need a DOT physical even if you don’t cross state borders.

Typically, a DOT physical every 24 months to maintain your qualification to drive a CMV. Depending on your medical history, more frequent exams might be necessary. If you meet the required medical standards, you will receive a medical examiner’s certificate, often referred to as a medical card.
