Opportunity Summary:

The Physical Therapist is responsible for providing comprehensive physical therapy services to patients of Clinton County Medical Center. This position evaluates patient conditions, develops individualized treatment plans, implements evidence-based interventions, and documents services provided. The Physical Therapist works collaboratively with patients, providers, and interdisciplinary team members to promote functional improvement, restore mobility, reduce pain, and improve quality of life while supporting the organization’s Patient-Centered Medical Home (PCMH) model. Schedule Requirement: Candidates must be available to work a minimum of two evening shifts per week as part of their regular schedule.

Essential Duties:

  • Evaluate patients and develop individualized plans of care based on clinical findings and patient goals.
  • Provide physical therapy treatment using evidence-based practices and professional standards of care.
  • Assess patient progress and modify treatment plans as needed to achieve optimal outcomes.
  • Educate patients and caregivers regarding diagnoses, treatment plans, home exercise programs, and preventive strategies.
  • Maintain accurate, timely, and complete documentation of evaluations, treatments, progress notes, and discharge summaries.
  • Collaborate with physicians, advanced practice providers, rehabilitation staff, and other healthcare professionals to coordinate patient care.
  • Ensure compliance with organizational policies, professional practice standards, payer requirements, and applicable regulations.

Requirements:

  • Strong clinical assessment and treatment planning skills.
  • Knowledge of evidence-based physical therapy interventions across a variety of patient populations.
  • Excellent communication and interpersonal skills.
  • Ability to collaborate effectively within an interdisciplinary healthcare team.
  • Strong organizational, analytical, and problem-solving abilities.
  • Commitment to patient-centered care and customer service excellence.
  • Doctor of Physical Therapy (DPT) or Master of Science in Physical Therapy (MSPT) degree from an accredited physical therapy program required.
  • Active Michigan Physical Therapist License required.

Preferences:

  • Previous clinical physical therapy experience preferred.
  • Experience working within an interdisciplinary healthcare environment preferred.

Physical Requirements:

  • Frequent standing and walking throughout the workday.
  • Frequent bending, reaching, stooping, and repetitive motion.
  • Ability to assist patients with transfers, mobility, and therapeutic activities.
  • Frequent use of treatment equipment, exercise equipment, and computers.
  • Ability to lift and move up to 20 pounds regularly and greater amounts occasionally with appropriate assistance or equipment.
  • Ability to communicate effectively with patients, caregivers, and healthcare professionals.
